Application (2 Set) Photo of Husband and Wife (Joint Photo) 3'X2' (Simple and latest with white background) Should be pasted on both application. 4. Affidavit from boy..............................(Self Attested) (Should be pasted Passport Size Photo) Simple and Latest. 5. Age Proof Boy (Self Attested) Only 10th Certificate and Birth Certificate. 6. Address proof Boy (Self Attested) Only Voter Id Card/Passport/Driving License/Ration Card. 7. Affidavit From Girl (Self Attested). 8. Age Proof Girl (Self Attested) Only 10th Certificate and Birth Certificate. 9. Address proof Girl (Self Attested) Only Voter Id Card/Passport/Driving License/Ration Card. 10. Affidavit of Witness from Boys (Passport Size Photo with ID) Simple and Latest (Pref. Father or Mother)   11. Affidavit of Witness from Girl (Passport Size Photo with ID) Simple and Latest (Pref. Father or Mother) 12. Affidavit  from Pandit/Kazi/Padri/Granthi (Passport Size Photo with ID) 13. Certificate/Bill/Document from marriage held in................. 14. Marriage card (Both Side) 15. Marriage Photo (4 Nos.) Postcard Size (With Clear Faces Both) 16. Affidavit, if Delay i.e. after 90 day's (Joint Affidavit Boy and Girl with Photograph, White Background) Simple and latest. 17. Photo of the bride applying vermillion on her forehead. 18. Photo of the bride wearing Mangalsutra around her neck. 19. Photo while circling with the bride. Note:- 1.Address Proof in all application and affidavit should be matched by gives ID's.            2. Please carry your all original document during attendance in Joint Commissioner Office.             3. File should be tagged properly and in above mentioned order.
